Pupil Medical Information Form

This form is to be completed by all parents and carers of children with a medical condition.

Consent to Administer Medicines

This form only needs to be completed if the child needs to be receive medication during the school day. We will not be able to give your child their medicine unless you complete and ......the declaration at the end.

School Asthma Health Plan

This form is to be completed if your child has a diagnosis of asthma. Please provide a blue inhaler and spacer that can be kept in school. Both of these items should be labelled. Your child's blue inhaler should have the dispensing label on it and these items should be brought into school in a small clear bag which can be sealed.
